Applications are invited for the temporary position of Clinical Specialist Dietitian – Diabetes Integrated Care in the Nutrition and Dietetics Department of St. Vincent’s University Hospital.
Role Summary: To provide integrated care across CHE as part of the integrated care structure for patients with Type 1 diabetes/complex Type 2 diabetes. Integrated care requires health and social care services to work together across different levels and sites in order to provide care that meets patient needs.
Key Duties and Responsibilities:
• Ensure appropriate care pathways are developed with a focus on patient-centred service, planning to ensure services are built around patients.
• Support timely access to all health & social care services according to medical need.
• Work in collaboration with other team members in co-ordinating and developing services to meet the need of patients with Type 1 Diabetes in the region.
• Responsible for delivery of high quality dietetic service in accordance with standards of professional practice.
• Carry out clinical and educational duties as required.
• Work with the Dietitian Manager in ensuring co-ordination, development and delivery of a quality, patient centred dietetic service across and between hospitals and networks in the region.
