Applications are invited for the permanent position of CNM2 Infectious Diseases/Sexual Health in St Vincent's University Hospital.
Role Summary:
The role of the Clinical Nurse Manager 2 is to facilitate a coordinated in-reach & out-reach approach to caring for vulnerable patients attending SVUH. They will ensure that multidisciplinary and multi-agency inputs are synchronised and targeted to match individual (acute and post-acute) care needs. Knowledge of the nature of the patient’s life situation and identifying and matching services in response to their health and welfare requirements is an essential element of the role. The role will be placed within the Infectious Diseases department, where the priority will be for persons at risk of infection.
