Description
Applications are invited for the position of Senior Pharmacist - Medicines Optimisation in the Pharmacy Department of St. Vincent’s University Hospital.
Purpose of the post: Coordinate and support medicines optimisation initiatives which promote safe, effective and efficient prescribing within the hospital. Advise and provide information to other healthcare professionals, including other pharmacists, to assist in pharmaceutical care and the implementation of local and national medicines optimisation initiatives. Identify and address medication management and patient care needs where appropriate.
