Description
Applications are invited for the position of Clinical Nurse Specialist - Rheumatology in St. Vincent’s University Hospital.
Role Summary: In
conjunction with the Consultant Rheumatologists the post holder will provide a
service, which optimises quality and continuity of care between primary and
secondary care. The post holder will provide expertise and specialist nursing
services to patients and their families, both in the acute hospital and PCCC settings. The
primary focus of the post holder will be to provide specialist guidance to
patients and care providers to reach their full potential to remain or return
to their homes and community units and avoid inappropriate admission to
hospital.
In developing and
establishing the service, the post holder will provide nursing expertise,
education and a management consultation service to patients and relatives, via
nurse lead clinics, referrals, integrated care protocols and patient quality
initiatives.
