Applications are invited for the position of Clinical Nurse Specialist – Heart Failure in St. Vincent's University Hospital.
The in-patient phase of heart failure care begins with emergency admissions to either St. Vincent’s Hospital or St. Michaels Hospital. The outpatient heart failure department is located in St. Michael’s Hospital and there are more than 400 outpatient consultations per month with the multidisciplinary care team at St. Michael’s Hospital. The patient age group averages 78 years.
The clinical nurse specialist acts as a co-ordinator of care within the multi-disciplinary heart failure team. The service aims to prevent repeated readmissions associated with heart failure and to provide a high quality holistic care for patients post heart failure diagnosis. The service has expanded over the years to include rapid access for GP’ in the area for heart failure patients. The unit strives to develop ways to improve patient management throughout the continuum of care required for those with a diagnosis of heart failure.
The purpose of the Clinical Nurse Specialist Heart Failure post is to:
Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.
The CNS Heart Failure will focus initially on the following patient groups applicable to their scope of practice:
• Patients referred to the service’s New Diagnostic Clinic with a possible diagnosis of heart failure
• Patients with a diagnosis of heart failure referred by another service
• Patients following an admission to hospital with acute decompensated heart failure
• Patients referred to the service with a known diagnosis of heart failure for titration of disease modifying therapy
